About Us

Health Choice Generations is a Medicare Advantage Health Maintenance Organization (HMO) Special Needs Plan (MA-PD) contracted with the Centers of Medicare and Medicaid Services (contract H5587). This plan is designed for people who meet specific enrollment criteria. This includes anyone who receives medical assistance from the State and Medicare.

To join the Health Choice Generations plan you must live in one of the nine counties in the service area covered by Health Choice Generations: Apache, Coconino, Maricopa, Mohave, Navajo, Pima, Pinal, Santa Cruz and Yuma.

According to the rules and regulations of The Centers for Medicare and Medicaid Services (CMS), Health Choice Generations may refuse to renew its contract with CMS; CMS may refuse to renew it's contract with Health Choice Generations; and the termination or non-renewal of Health Choice Generations contract may result in member's disenrollment from the plan. Health Choice Generations may also reduce its service area and no longer offer services in the areas where you reside. If this happens, we will notify you - in writing - 90 days in advance.

Health Choice Generations is an affiliate of Health Choice Arizona, Inc. Health Choice Arizona's parent company, IASIS Healthcare, owns and operates 14 general, acute-care hospitals, one behavioral hospital and three ambulatory surgery centers in Arizona, Florida, Nevada, Texas and Utah.
