Welcome to Health Choice Generations!

In the Provider area of the HC Generations Web site, you will find tools and resources to help serve your Medicare patients.

Health Choice Generations looks forward to partnering with you to deliver better outcomes, increased patient satisfaction and preventive care. Let us know how we can help.

About Health Choice Generations Network Services

The Health Choice Generations Network Service Department is responsible for putting together and maintaining the Health Choice Generations provider network of primary care physicians, specialty care physicians, hospitals, pharmacies, and other types of providers to service our members.

Each provider is assigned a Network Service Representative. The Network Service Representative serves as an important link to our health plan. The Representatives have many responsibilities, but the most important is to insure that we have a network of providers that can meet the needs of our members.

The Representatives have three major roles:

  1. Educate provider/office staff
  2. Add new providers to the health plan when needed
  3. Insure the providers are in compliance with his/her contract agreement with Health Choice Generations and resolve issues.
