Checking Member Eligibility
Health Choice Generations offers Providers two ways to check eligibility:
- Accessing the Health Choice Generations Web site Provider Log-In/Sign-Up.
All providers are required to pre-register on the Web site to gain access for member eligibility. If you need assistance, please contact your Network Services Representative.
- Calling Health Choice Generations Member Services, 8 am - 8 pm, 7 days a week (except holidays) at (800) 656-8991.
Providers should check member eligibility at each point of contact, as eligibility can change at anytime. The Member's ID card covers both Medical and Prescription benefits. All providers including pharmacies should ask to see the Health Choice Generations Member ID card.
If a patient's eligibility status terminates with (AHCCCS) and they are no longer enrolled in AHCCCS, then the member's eligibility status can change from Health Choice Generations after 60 days from the disenrollment with AHCCCS.
