Transition of Care in the Case of a Terminated or Non-Contracted Provider

Health Choice Generations will allow a 60 day transition of care for members from a terminated practitioner to a contracted provider (PCP or specialist). Members will receive a letter from Health Choice Generations with at least a 60 day advance written notice. The terminating provider and the accepting provider will communicate all health care treatment to ensure continuity of care for the member. In some areas where there are limited specialty providers, Health Choice Generations may allow a non-participating provider to continue care if a member is under active treatment.
