Why should I join Health Choice Generations?

Why Health Choice Generations HMO Medicare Advantage Plan is a healthy choice for you.
The plan has the features that matter most to you when it comes to prescription drugs, including:

  • No annual deductible or monthly Part D premium - you could start saving money as soon as you get your card.
  • You will pay $0 to $2.50 for generic, retail and mail-order prescriptions and $0 to $6.30 for brand-name, retail drugs and mail-order prescriptions depending on your resources and income.
  • Mail Order Prescription Drug Benefit, 90-day supply shipped right to your home saving you time and money.
  • Access to thousands of drugs allowed by Medicare - which means the medications you use today are more than likely covered.
Health Choice Generations members - through our Pharmacy Benefit Manager, Express Scripts - contracts with pharmacies that equals or exceeds CMS requirements for pharmacy access in your area that total to over 54,000 network pharmacies nationwide. These pharmacies include retail, mail order, long-term care, home infusion and I/T/U (Indian Health Service, Tribes or Urban Indian) pharmacy services - you could fill your prescriptions at both large national chains and neighborhood pharmacies.

We encourage our members to inform Member Services right away if they have questions, concerns or problems related to covered services, healthcare or require assistance with problem solving.

Please call us at 1-800-656-8991, 8 am - 8 pm, 7 days a week. TTY users should call 1-800-842-4681. Calls made to both these numbers are free.
